DECLARATION FOR AN ORIGINAL INVALID PENSION.

On the 30th day of September, A. D., one thousand eight hundred and seventy-nine personally appeared before me John G. Hoffman, Clerk of the Circuit Court of Record within and for the county and State aforesaid, Alonzo Bockus age 39 years, who being duly sworn according to law, declares that he is the identical person who was ENROLLED on the 16th day of Sept., 1861, in company AG= of the 45th regiment of Pa Vols commanded by Nelson Whitney, and was honorably DISCHARGED at Otter Island on the 25th day of March, 1862, that his personal description is as follows: Age, 39 years; height, 5 feet 8 inches; complexion, light hair, auburn; eyes, gray. That while a member of the organization aforesaid, in the service and in the line of his duty at Otter Island, in the State of So. Ca., on or about the _____ day of December, 1861, he had an attack of measles, caught cold settled on his lungs resulting in Disease of the Lungs.

That he was treated in hospitals as follows: by Regt. Surgeon.

That he has not been employed in the military or naval service otherwise than as stated above.

That since leaving the service this applicant has resided in the Town of Lowell, in the State of Ind. and Tioga Co., Pa., and his occupation has been that of a farmer. That prior to his entry into the service above named he was a man of good, sound, physical health, being when enrolled a farmer. That he is now partially disabled from obtaining his subsistence by manual labor, by reason of his injuries, above described, received in the service of the United States; and he therefore makes this declaration for the purpose of being placed on the invalid pension roll of the United States. He hereby appoints with full power of substitution and revocation, _________

his true and lawful attorney to prosecute his claim. That he has not received nor applied for a pension; that his residence is No. ____________ and that his post office address is: Lowell, Lake Co., Ind.

CERTIFICATE OF DISABILITY FOR DISCHARGE.

Alonzo Bockus ((Private), of Captain Nelson Whitney=s Company (G,) of the Forty-Fifth Regiment of Penna. Vols., was enlisted by Nelson Whitney of the Forty-Fifth Regiment of Penna. Vols., at Charleston Tioga Co. On the Twenty Sixth day of September, 1861, to serve during the war, he was born in Charleston in the State of Pennsylvania, is Twenty-One years of age, Five feet 8 l/2 inches high, light complexion, grey eyes, light hair, and by occupation when enlisted a farmer. During the last two months said solder has been unfit for duty sixty days.

Date: March 22, 1862 ) Co. G 45 Regt. Penna. Vols.

Commanding Company.

I certify, that I have carefully examined the said Alonzo Bockus, Private of Captain Nelson Whitney=s Company, and find him incapable of performing the duties of a soldier because of: chronic disease of the lungs, which renders him unfit to perform military duty.

Personally appeared Esther Rockwell of Hebron, P.O., County of Porter, State of Indiana, who being duly sworn upon her oath declare as follows: That she was present at the time of the birth of Rosella Bockus on the 10th of November, 1869 and said affiant further states she is Aunt to the said Rosella Bockus.

And she further say that the knowledge of the above facts is obtained from the following sources, viz: she was present

and that she has no interest or concern in the matter.

Subscribed and sworn to before me, this 17 day of July 1891, and I certify that the party whose name appears signed to the foregoing affidavit is the person she represented to be and is a good and credible witness, and that the contents of the foregoing affidavit were duly read and fully known to affiant before making oath to the same, and that I have no interest in the matter.

DECLARATION FOR late WIDOW=S ARMY PENSION

On the 23rd day of February, 1891, personally appeared before me, a Clerk of the Court for the County and State aforesaid, Amy Smith a resident of Lake County, State of Indiana, aged 51 years, who being first duly sworn according to law, makes the following application for pension under the laws of the United States, approved July 14, 1862: That she is the late lawful widow of Alonzo A. Bockus who served in the service of the United States as a member of Company G 45th Regiment of Penn. Vol., in the war of 1861. He enlisted at Penn. On or about the ____ day of Sept., 1861, at ________. While in said service he contracted disability which resulted in death at Lowell, Ind. On the 6th day of July, 1880. That she was married to him on the 12th day of Nov., 1862, in the County of Tioga, State of Penn. And her name before marriage was Amy Kelsey. That she has remarried, since his death to wit: on the 22nd of August, 1883. That at the date of his death he left children under the age of sixteen years, whose names and ages are as follows, viz:

Estella Bockus born on the 4th day of Sept. 1867.

Rosella Bockus born on the 10th day of Nov. 1869.

Nellie Bockus born on the 4th day of June 1871.

Etta May Bockus born on the 15th day of Feby. 1880.

That the above are the only legitimate children of herself and said husband now living who were under the age of sixteen years. That she has remarried as aforesaid and has not abandoned the support of said children. That he said husband left no child or children by any prior marriage. That she has not been engage in nor aided or abetted the rebellion in the United States. She appoints P. H. Fitzgerald of Indianapolis, Ind., her true and lawful attorney, without revocation, to prosecute this claim for pension.

She requests her pension to be paid at the nearest Agency.

Her P.O. is Lowell County of Lake State of Indiana.

On THIS Fourth day of January A.D. 1882; personally appeared before me a Notary Public in and for the aforesaid County, duly authorized to administer oaths T. V. Rockwell aged 49 years, a resident of South Chicago in the County of cook and State of Illinois whose Post Office address is South Chicago Ill and Esther Rockwell aged 42 years, a resident of South Chicago in the County of Cook and State of Illinois whose Post Office address is South Chicago Ill. Well known to me to be reputable and entitled to credit, and who, being duly sworn, declare in relation to aforesaid case as follows:

The above T. V. Rockwell and his wife Esther Rockwell deposeth and sayeth that they were personally acquainted with and had known the above Alonzo Backus since he was a boy and knew him to be healthy before he went in the U. S. Army. We are acquainted with the fact that when in the army he was taken sick with the measles, he was taken from the Hospital before he became sufficiently well, and it settled in his lungs and we know that he had not seen a well day from the time he returned from the army until he died.

On the 23rd day of March A. D., one thousand eight hundred and 92 personally appeared before me Notary Public within and for the County and State aforesaid, Amy Smith a resident of Lowell in the County of Lake and State of Ind., aged 52 years, who being first duly sworn according to law, doth, on oath, make the following declaration, as guardian of the minor child of Alonzo A. Bockus deceased, in order to obtain the benefit of the provisions made by the Act of Congress, approved July 14, 1862, granting pensions to minor children under sixteen years of age, of deceased officers and soldiers. That she is the guardian of Etta May Bockus

(These being the only children of said deceased under sixteen years of age), whose father was a member in Company G. Commanded by Captain ____________ in the 45 regiment of Penn. Vols. In the war of 1861;_______________ who while in the service aforesaid, and in the line of his duty contracted disability which resulted in his death at Lowell in the State of Ind. On the 6th day of July 1880. That the mother of the child aforesaid remarried on the 22 day of Aug. 1883, and that the date of the birth of her Ward and her residence is as follows, to-wit: Etta May Bockus born on the 15 day of Feby 1880.

She further declares that the parents of his ward were married at Covington in the County of ________, State of Penna. On the 12 day of Nov. In the year 1862, by Rev. U. L. Reynolds. She also declares that neither she nor her ward have in any manner been engaged in or aided or abetted the rebellion in the United States. She hereby appoints Fitzgerald & Vegan of Indianapolis County of Marion State of Indiana.

Personally appeared John E. Carpenter of Round Top P.O., County of Tioga State of Pa. Who, being duly sworn upon his oath declare as follows:

I, John E. Carpenter, have been acquainted with Alonzo A. Bockus since my earliest recollection Our fathers farms joined we grew up together and as far as I knew he enjoyed as good health as any of the boys. I never knew him to have a sick spell until he returned from the Army. In the summer of Fifty-Nine (59) 1859 I worked with Alonzo A. Bockus doing haying and harvesting and he was able to do as much work as any man. I never heard of his being a sickly man before going to the Army.

I John E. Carpenter was born January 23, 1845.

And he further say that his knowledge of the above facts is obtained from the following sources, viz: personal acquaintance

and that he had no interest or concern in the matter.

From childhood almost to manhood I lived within less than half mile of Alonzo A. Bockus so remember he was as healthy as the great majority of young men. I do not remember of his ever being sickly. I( knew him very intimately--going to the same district school. I have known nothing o him sick his enlistment

and he further say that his knowledge of the above facts is obtained from the following sources, viz: having been acquainted with him since childhood and that he had no interest or concern in the matter.

In the matter of the claim of Amy Smith, former widow of Alonzo Bockus, Priv. Co. G--45 Regt. Pa. Vols.

Personally came before me a Notary Public in and for the County and State aforesaid Estella Mahler, aged 50 years; 2958 E. 80th Place, Chicago, Ill., person of lawful age, who, being duly sworn, declare in relation to the aforesaid claim, as follows:

The applicant in this case is my mother. My late father, the soldier died in the year 1880, and I know that the applicant did not re-marry until her marriage to John Smith, about three years later. She and the said John Smith lived together as husband and wife from the time of their marriage until his death in the year 1905, being at no time separated or divorced from each other. She has not remarried since the death of the said John Smith.

I was present and witnessed the funeral of the soldier, which occurred at Lowell, Indiana, and also that of John Smith, which also took place at Lowell, Indiana.

I make the above statements from intimate knowledge, and close association with the parties mentioned.

I have no interest in this case.

In the mater of the claim of Amy Smith, formerly widow of Alonzo Bockus, Co. G, 45th Pa. Vols.

Personally came before me a Notary Public in and for the County and State aforesaid Wilford Stratton, Sr., aged 70 years, Lowell, Indiana.

I knew the soldier and applicant in this case intimately from the time of their marriage in 1862 until his death in or about July 1880, and I know that they lived together and cohabited as husband and wife continuously during that time, and were so recognized by the community in which they resided, and were not divorced from each other. We visited back and forth and were intimately acquainted and associated with each other during said period and I would know or have heard something of the fact had a divorce been granted to either party.

I have no interest in this case.
