State of Pennsylvania, County of Bradford_ _ _ _ _ _ _ _ _ _ _ ss:
On this Twenty first day of February, 1885, personally appeared Eliza
J. Van Orman, who, being duly sworn, declares that she is the lawful widow
of William D. Van Orman, deceased; that he died on the 20th
day of November, 1884; that he had been granted a pension by Certificate
No. 181.227 which is herewith returned; that he had been paid the pension
by the Pension Agent at Philadelphia up to the 4th day of September,
1884; after which date he had not been employed or paid in the Army, Navy
or Marine service of the United States, that she was married to the said
William D. Van Orman on the Seventh day of December, 1862, at Warren Township,
Bradford Co., in the State of Pennsylvania; that her name before said Marriage
was Eliza J. Corbin; that she hereby makes application for the pension
which had accrued on aforesaid certificate to the date of death; and that
her residence is Orwell Township in the County of Bradford, State of Pennsylvania,
and her Post office address is Potterville, Penna.

Also personally appeared S. M. Beecher and I. M. Hunt, residing at
Potterville, Bradford County, Pa, who, being duly sworn, say that they
were present and saw Eliza J. Van Orman sign her name to the forgoing declaration;
that they know her to be the lawful widow of William D. Van Orman, who
died on the 20th day of November, 1884; and that their means
of knowledge that said parties were husband and wife, and that the husband
died on said date, are as follows: Have been well acquainted with the said
William D. Van Orman and Eliza J. Van Orman for several years, and know
that they lived together as Man and Wife, and that they were present at
the funeral of the said William D. Van Orman on the 22nd day
of Nov., 1884. who died on the 20th day of Nov., 1884.

February, 1885, and I certify that the affiants are reputable persons;
that they know the contents of their depositions, and that their statements

APPLICATION FOR A NEW CERTIFICATE
|

State of Pennsylvania, County of Bradford_ _ _ _ _ _ _ _ _ _ _ ss:
On this 31st day of July, 1908, before the subscriber,
a Justice of the Peace for said county, personally appeared Eliza J. Van
Orman, who on oath declares that she is 63 years of age and the widow of
William D. Van Orman, who belonged to Company D. in the 5th
Regiment of New York Cavalry, in the service of the United States, in the
war "Civil"; that her name was placed on the pension roll of the State
of Pennsylvania, that she has not bartered, sold, assigned or pledged her
pension certificate, or any interest therein; but that on or about the
___ day of Dec., 1907, at or near Stevensville being the last time she
saw it and thinks it was left at Fred Fasset’s store and the store and
contents were all burned soon after.
My post-office address is LeRaysville, Bradford County, Pa.

Also, personally appeared Charles H. Hutchinson, who on oath declares
that he well knows Eliza J. Van Orman, who has executed the foregoing affidavit,
to be the identical pensioner named herein.
