County of Bradford ss:
On this 23rd day of August, A.D. one thousand eight hundred
and seventy-seven personally appeared before me, the prothonotary of the
Court of Common Pleas of Bradford County, the same being within and for
the county and state aforesaid, William D. Vanorman, aged 41 years, a resident
of the Township of Orwell, county of Bradford State of Pennsylvania, who
being duly sworn according to law, declares that he is the identical William
D. Vanorman who was ENROLLED on the Eleventh day of February, 1864, in
company D of the Fifth regiment of the Eighteenth New York Cavalry commanded
by Lieut. Appleby, and was honorably DISCHARGED at Philadelphia, Penna
on the fifth day of August, 1865; that his personal description is as follows:
Age 41 years; height, 5 feet 7 inches; complexion, dark; hair black; eyes,
Hazel. That while a member of the organization aforesaid, in the service
and in the line of duty at White Horse Landing, in the State of Virginia
on or about the 20th day of June, 1864, he was thrown from his
horse into the river and caught cold which resulted in inflammation of
the kidneys.
That he was treated in hospitals as follows: Cavalry Corps Hospital
at City Point, Va., Mc Dougal Hospital, N. Y. Harbor. St Mary’s Hospital,
at Rochester, N. Y.
That he has been employed in the military or naval service otherwise
than as stated above viz., he did hospital duty at Fort Sumner Washington,
D.C. and was there taken worse and was sent to Campbell Hospital and there
became better and was transferred to Company F, 20th Regt. Vet
reserves on April 13, 1865; and did service in that corps until he was
discharged as aforesaid.
That since leaving the service this applicant has resided in the
Counties of Wyoming and Bradford in the State of Penna., and his occupation
has been that of a farmer and shoemaker.
That prior to his entry into the service above named he was a man
of good, sound, physical health, being when enrolled a farmer. That he
is now wholly disabled from obtaining his subsistence by manual labor by
reason of his injuries, above described, received in the service of the
United States; and he therefore makes this declaration for the purpose
on being placed on the invalid pension-roll of the United States.
He hereby appoints, with full power of substitution and revocation,
Nathan W. Fitzgerald of Washington, D.C., his true and lawful attorney
to prosecute his claim. That he has not received nor applied for a pension.
That his POST OFFICE ADDRESS is; West Warren, County
of Bradford, State of Pennsylvania.

Also personally appeared Cyrus Cook, residing at Orwell Twp., Bradford
Co., Penna. And Josiah J. Newell, residing at Orwell Twp. aforesaid, persons
whom I certify to be respectable and entitled to credit, and who, being
by me duly sworn, say they were present and saw William D. Vanorman, the
claimant, sign his name (or make his mark) to the foregoing declaration;
that they have every reason to believe, from the appearance of said claimant
and their acquaintance with him, that he is the identical person he represents
himself to be; and that they have no interest in the prosecution of this
claim.
Sworn to and subscribed before me this 23rd day of August,
A.D. 1877, and I hereby certify that the contents of the above declaration,
&c., were fully made known and explained to the applicant and witnesses
before swearing, including the words erased and the words added; and that
I have no interest, direct or indirect, in the prosecution of this claim.
STATE OF New York, COUNTY OF Tioga }ss:
Be it Known that on this 12th day of March, 1878 before
me, Jno. C. Gray, Clerk of Tioga County and Clerk of the Supreme Court
of the State of New York in and for said county personally Daniel M. Pitcher
and J.B.G. Babcock of the Village of Owego in said county to me known and
whom I certify to be reputable and worthy of credit, who being duly sworn
each for himself deposes and says that he was well acquainted with William
D. Vanorman, who enlisted as a Private in Company D, 5th Regt.,
N.Y. Vol. Cav. on the 11th day of February 1864, that they were
well acquainted with him at the time of his said enlistment and for the
five years immediately preceding the same, that during that time they saw
the said Wm D. Vanorman almost every day, and he seemed and to all appearance
was in perfect health and sound physical condition, and to the best knowledge
and belief of deponents he was in good health and physically sound during
all that time and at the date of his enlistment. Deponents further say
that they have no interest in the prosecution of this claim.

In the matter of the claim for invalid pension No. 242897 of William
D. Van Orman, Personally Came before me, a Notary Public in and for aforesaid
County and State, Henry Basford a citizen of Owego, County of Tioga, State
of New York, well known to me to be reputable and worthy of credit, who
being duly sworn, declares in relation to aforesaid case as follows: That
he has been personally acquainted with William D. Van Orman for about 19
years, that at the time of the enlistment of said Van Orman he lived a
near neighbor to deponent and deponent saw him nearly every day. And at
the time of said enlistment said Van Orman was of good health and sound
physical condition, being free from kidney disease and from diseases of
all kinds. Deponent further says that immediately after his discharge said
Van Orman was troubled with kidney disease and received treatment therefore
from that time up to 1970 from which time to the present date deponent
has seen said Van Orman occasionally and on those occasions said Van Orman
has been and still is suffering from said kidney disease. Deponent’s Post
Office address is Owego, Tioga County, N.Y. He further declares that he
has no interest in said case, and is not concerned in its prosecution.

prior to his enlistment, and worked in the same shop up to his enlistment,
That at the time of his enlistment said Van Orman was of good health and
sound physical condition, being free from kidney disease and from diseases
of all kinds. That at the time of his discharge and for one year thereafter
this deponent and said Van Orman worked in the same shop, and said Van
Orman was troubled with kidney disease and received treatment therefore
during all that time. Deponent’s Post Office address is Owego, Tioga County,
N.Y. He further declares that he has no interest in said case, and is not
concerned in its prosecution.

Be it Known that on this 25th day of June, 1879 personally
appeared before me, a Notary Public in aforesaid County and State, Wm H.
Camp whom I certify to be reputable and entitled to credit, who being duly
sworn says that he was a member of Company D, 5th Regt of N.Y.
Vol. Cavalry and that Wm D. Van Orman was a member of said organization,
That he was taken sick and that deponent saw him in the hospital at City
point, Va. On or about the first of July 1864, and understood that he was
suffering from kidney disease. Deponent further said that6 he was well
acquainted with said Van Orman previous to his enlistment and worked in
the same shop with him. That he was in sound health and free from disease
of the kidneys. That he knows these facts from personal observation and
that he has no interest in this case, and is not engaged in its prosecution.
Subscribed & sworn to before me this 25th day of June
1879. And I certify that the contents of the foregoing affidavit were made
known to the witness before execution, and that I am not interested in
the prosecution of this claim.

Have been sick the greater portion of the time ever since have
doctored much with only present relief. For the last four years of my life
I have not been able to do any kind of labor and most of the time not able
to wait upon myself. I was said to have inflammation of kidneys or kidney
disease at all places and by all doctors except at St. Mary’s Hospital
Rochester New York.
After I was transferred to veterans reserve corps regt only done
light duty until discharged. I was strictly under charge of the head surgeon
of Cavalry Corps Hospital, but his name or whereabouts I am unable to acertain.

In the matter of Invalid Pension No. 242897 of Wm D. Van Orman, Co.
D, 5th Regt., New York Cavalry, personally comes the affiant
who being first sworn, on oath says: That he is a resident of the township
of Orwell in the county of Bradford and State of Pennsylvania, and has
been for the last eighteen years, and has been well and intimately acquainted
with Wm D. Van Orman late a private of Company D, 5th Regt.,
New York Cavalry Vols., ever since his discharge from the service having
lived in the same township with him, and worked with him in the same shop.
"I know that he complained of his back and had faint times which was called
heart disease. I know he continued to complain of his back and heart and
finally got so bad he could not labor but a very little, and for the last
five years could scarcely do any work and has not been expected to live
for a long time in consequence of heart disease and kidneys." And affiant
further swears that he is not interested in the prosecution of the claim,
and his Post Office address is Potterville, Co. of Bradford, State of Pennsylvania.
Subscribed and sworn to before me this 18th day of June,
1880. The affiant is the person he represents himself to be, and a credible
witness. I am not interested in this claim sought to be established. Witness
my hand and seal, day and year above written.

In the matter of Invalid Pension No. 242897 of Wm D. Van Orman, Co.
D, 5th Regt., New York Cavalry Vol personally comes the affiant
who being first sworn, on oath says: That he is a resident of the township
of Orwell in the county of Bradford and State of Pennsylvania, and has
bin for most of the time for the last twenty years and has been well and
intimately acquainted with Wm D. Van Orman late a private of Company D,
5th Regt., New York Cavalry Vols., ever since his discharge
from the service having lived a neighbor most of the time, seeing him often
and consider that he has not bin well since leaving the service. He has
complained most of his back, said to be kidneys. About five years ago he
was atackted more severe and has bin unable to do but little labour since
and his disease is said to be hart and kidneys disease. I see the aplycant
as often as twice a week and his case is said to be Critical. And affiant
further swears that he is not interested in the prosecution of the claim,
and his Post Office address is Potterville, Co. of Bradford, State of Pennsylvania.

D, 5th Regt., New York Cavalry Vol personally comes the affiant
who being first sworn, on oath says: That he is a resident of the township
of Warren in the county of Bradford and State of Pennsylvania, and has
bin a resident of Orwell and Warren ever since his birth. Have bin aquainted
with said Wm D. Van Orman since his discharge from the service. Was with
him quite often before he enlisted and consider he hasa never bin a well
man since his discharge. He complained of kidneys often and heart by spells,
but not so often. Inb the year 1874 I hiered the said Wm D. van Orman to
work for me one year and he had to lose much time on account of heart and
kidneys and finally gave out entirely and had to quit work his heart was
so bad and so much pain in back that he could not work at shoe making and
has done but little labour since and his death is Daily expected. And affiant
further swears that he is not interested in the prosecution of the claim,
and his Post Office address is Potterville, Co. of Bradford, State of Pennsylvania.
